Our services are designed for older adults and individuals with disabilities who want to live comfortably and safely in their own homes and communities.
Skilled Nursing Services
This service is available to individuals who present a medical need. Our licensed nurses will assess the individual to determine the level of care and type of services to be provided.
Respite Home Based / Out of Home Based
Our staff will provide personal care, companionship, and home support services to the client during the short or long term absence of the regular caregiver.
Elderly Care & Attendant Care
Direct Support Professionals assist clients with personal hygiene such as bathing, dressing, and grooming. They also provide support with mobility, toileting, meal preparation, and feeding, while ensuring comfort and safety.
Homemaker/Chore Services
Assigned staff will assist the client to complete chores in the home. Staff will also ensure the client’s living environment is safe, secure and sanitary.
Home & Community Habilitation
Direct staff will provide 1:1 support to the individual by assisting them with creating a resume, seeking employment, holding mock interviews, and providing on-the-job support as needed once employment is obtained.
Companion Care
Direct Care Staff will support individuals in the home or the community, ensuring the general health, safety, and well-being of their clients at all times in all environments.
Behavioral Supports
Our Behavior Specialists will collaborate with the treatment team to create and implement specialized interventions that would increase the individual’s adaptive skills and replace challenging behaviors. With understanding, we utilize a proactive approach to manage each behavior.
Community Residential Services
(Licensed under 55 Pa Code Chapter 6400) Community Residential Rehabilitation Services support individuals with intellectual disabilities in a structured, 24/7 supervised environment. Services focus on daily living skills, socialization, money management, safety awareness, and behavioral support to promote independence and community integration.
Competitive Employment
Our Employment Specialists will work with clients, providing ongoing assistance in developing and maintaining communication, socialization, self-esteem, and adaptive skills to remain employed in the community. Direct support Professionals will provide 1:1 care to individuals, assisting them in tasks that are a part of everyday living both in the home and in the community, teaching skills as needed to enable the individual to be as independent as possible.
